 Our Family Wellness Coaches can meet you anywhere that works best for you, at home, at the local coffee shop, at your children's recreational activities, so that you can receive the help you need wherever life takes you. 

Have questions regarding the recommendations shared with you during your weekly visit? Not a problem, your Family Wellness Coach and you will schedule a 15 mins follow-up phone call at the end of your weekly home visit.

With your Family Pathways package, you have access to your information, summaries of each visit with recommendations, strategies, and resources shared with you during each visit. 

Your Family Wellness Coach and you will design a success road map with specific goals and steps to achieve your vision of family wellness using metrics, standardized assessment tools and consolidated information which will be available to you at the end of each package cycle.  This final assessment report will include all the pertinent information on your progress and achievements, along with all the recommendations and resources shared with you during your time with Family Pathways.
